UNM HPC Equipment Checkout Policy

Policies and Procedure Edition 1.3
UPDATED July 23, 2004

All equipment belonging to UNM HPC that leaves the premises must be checked out. To check out equipment from HPC, one must complete the UNM HPC Equipment Checkout Form. This form must be filled out in its entirety, then signed by the HPC Director or Deputy Director.

Another form may be relevant; there exists a official UNM Equipment Checkout Form. This form must also be completed if the equipment is going to leave the UNM campus. This includes cases where equipment that is already checked out to another department is going to leave campus.

The line detailing the purpose for checkout MUST be completed.

There will be a return date associated with all HPC equipment that has been checked out. This return date is ONE YEAR from the date of checkout, unless otherwise specified on the form. Exceptions for indefinite loans will be noted as the form is signed by the HPC Director or Deputy Director.

It is the responsibility of the borrower to care properly for the equipment and to return the equipment by the return date. An equipment loan may be extended by filing a new HPC equipment checkout form.

Equipment becomes due immediately if:

  • The purpose for checkout is complete
  • The borrower leaves UNM
  • The borrower changes departments at UNM
  • HPC deems it necessary that the equipment be returned
